Enrolment
We welcome new patients and new families moving to the area to enrol with us.
If you choose to enrol with Te Puke Medical Centre your fees will be less because we will receive a partial government subsidy for you.
You can only be enrolled at one medical practice, however you are free to attend any practice as a casual patient where you will pay the full unsubsidised fee.
If you wish to enrol with Te Puke Medical Centre please download the enrolment form here fill them in and give them to our receptionist. (There are 3 pages which include the enrolment forms and transfer request form). Please ensure our Terms and Conditions are read prior to signing.
Please note, you are required to bring in the appropriate identification for eligibility and New Zealand citizenship at the time of enrolling. You will need a separate form for each family member.

Fees
Standard Fees Monday – Friday as from 12th August 2024 These fees are for same-day settlement and include GST. We require full payment on day of consultation, and can be paid via cash, EFTPOS or credit card (fees may apply). Bank transfer also available, Please ask reception for account details and use invoice number as reference when processing.
Newly enrolled patients will have a ‘stand down’ charge (casual rate) for their initial consultation.

General Fee Information: Visitors are required to pay before their consultation. Additional charges will apply for longer appointments or if special equipment and/or materials are used. A full list of our current fees is displayed at the reception. There may be possible funding options for chronic health conditions, palliative care, skin surgery and mental wellbeing, please ask your GP for further information.
Subsidised Fees: You can apply for a Community Services Card (CSC) via WINZ (Ministry of Social Development) if you think you are eligible for subsidised fees.
